BLACK MOTHER Collective™ Community Care Navigation

This is the central community space for families engaged with The BLACK Collaborative Inc. Through Community Care Navigation, members receive coordinated support across our core programs, including BLACK Care™, Community Care Closet™, and BLACK R&B Heart Hub™.

This group provides:

• Program updates and announcements

• Upcoming event notifications (pop-ups, screenings, distributions)

• Resource sharing and referrals

• Direct connection to Community Health Workers (CHWs)

• Ongoing support for maternal, infant, and family wellness

This space is designed to ensure families stay connected, informed, and supported across all services.
