About

This training builds staff competency in identifying participant needs, selecting correct internal and external referral routes, and distinguishing medical referrals from social service referrals. Participants will learn to complete warm handoffs, document follow-ups, and prevent service duplication within the TBC closed HCAN HUB partner model. The curriculum focuses on ensuring resource navigation is accountable, documented, and participant-centered, covering essential pathways including Food Security, Housing, and Healthcare Coverage.
