The BLACK RHYTHM & BLUES (BLACK R&B) Heart Hub offers a Blood Pressure Monitoring & Loaner Program to help community members track and manage their blood pressure between medical visits.
Participants may receive education, blood pressure screenings, and temporary access to a home blood pressure monitor to support regular monitoring and heart health awareness.
This program supports the prevention and early detection of hypertension and cardiovascular risk.
