Cancellation Policy
📅 Appointments & Services: Appointments are available on Thursdays through The BLACK Collaborative Inc.’s Center of EDEN™. These visits help connect you to support, resources, and care coordination services. You may book an appointment for: New family intake Returning family support Diaper, food, or hygiene resources Mental wellness check-ins Health screenings Program referrals All services are non-clinical, meaning we do not provide medical care or treatment. 📝 Information & Consent: When you book an appointment, you agree to share basic information so we can support you. This may include: Your name and contact information Information about your children and household Your needs (such as food, diapers, or support services) Your information may be shared with trusted partners to help connect you to services, as part of care coordination. This follows the same guidelines described in our consent and release forms . You can ask questions at any time. 🔒 Privacy & Safety: We respect your privacy. Your information is kept secure, We only share what is needed to help you, We follow HIPAA and confidentiality rules, and You may choose not to answer any question you are not comfortable with. ⏰ Booking & Attendance Appointments must be booked in advance Please arrive on time If you are late, your appointment may need to be rescheduled If you cannot attend, please cancel or reschedule as soon as possible Follow-Up & Support- After your visit: A Community Health Worker (CHW) may follow up with you You may be contacted by phone, text, or email Follow-up helps us make sure you receive the support you need ⚠️ Service Availability - Services are based on: Availability of supplies Program eligibility Current demand Booking an appointment does not guarantee items or services, but we will do our best to support you. 🚨 Important Notes - These services do not replace medical care If you are in a medical emergency, call 911 If you need urgent mental health support, please seek immediate help ✅ Agreement- By booking an appointment, you understand and agree to: Share information needed for support Participate in care coordination services Be contacted for follow-up You may stop services at any time. 💬 Questions?If you have questions about your appointment or services, please contact our team.
